Here are some important terms for health insurance.

PPO: Preferred provider organizations, or PPO for short, is a managed care organization comprised of health care providers and doctors who provide health care at lower rates to a client’s insurer. This subscription-based medical care option offers big discounts below standard rates for doctors and other health care professionals who belong to the organization.

HMO: Health maintenance organizations, or HMO for short, are organizations that offer managed care for health insurance in partnership with hospitals and doctors. Patients need to have a primary care physician, from whom all referrals to specialized services must be approved.

Deductible: The amount that one must pay out of pocket before the insurance company will pay expenses.

Capitation: A set amount that a patient or the employer pays to an HMO whether or not the patient utilizes the services the HMO offers.

Co-insurance: Also known as a co-payment, this is the amount a patient is required to pay for a service after a deductible has been met. You may pay co-insurance each time you visit the doctor, for example.

  • What does this plan include?

  • What are the costs associated with this plan?
    In addition to your monthly premium, you'll likely have to pay other costs for your insurance. Some policies require you to make a co-payment when you visit the emergency room or your doctor. Others have a deductible, which you must pay before the health insurance will cover the cost of your claims.

  • Are there any maximums on this policy?
    Personal plans may limit how much each individual can claim per calendar year or how much your entire family can claim. Some policies also have lifetime maximums per illness or general caps on how much the policy will cover in total for as long as you have it.

  • Does this policy cover preexisting conditions?
    Some plans will not cover preexisting conditions for a set number of days, such as one to three months from the policy's effective date.

  • Can I choose my own doctor and am I free to see a specialist?
    Depending on your plan, you may have to select a doctor from a network approved by the insurance company. Care plans may also require you to get a referral from your primary doctor before seeing a specialist.
